Reporting and Ministry Submissions

Insight into agency performance is one of the most important benefits of collecting quality data. Often operational funding is directly related to clinical efficiency. We provide the full compliment of mandated reports that MOHLTC requires for funding submissions (CDS-MH and OHRS/MIS chapter 7).

Willow Tree provides many reports to determine the details of programs, workers, and clients:

• Program-based reports provide aggregate counts of important client demographic information such as age, gender, service location, presenting issues and various baseline/current measurements.
• Worker-based reports return valuable metrics of worker activities performed including client interaction details.
• Client reports focus on specific information such as client progress within a program, enrolment statistics, outcome measurements and worker contact methods/frequencies.
